Fill out this form after you've:

Read the Guidescope Affiliate Agreement and
Prepared and tested a graphic for the Guidescope Menu.

Submit your application by pressing "Agree & Submit". By submitting this form you are agreeing to the terms of the Guidescope Affiliate Agreement.

When you submit your application, we will give you a special Affiliate URL to use as a link to Guidescope on your site. Graphics for linking are here.

If you want to review or alter any of the information you give us on this form, you can do so at your password-protected Affiliate Member pages.

Affiliate Information

When you submit this form, we will send email to your Company Contact to confirm your application. That email will provide you with a temporary password to your Affiliate Member account management pages, where you can change your password.

The Company Contact may be an email aliases rather than a personal name, for example: "marketing@YourCompanysName.com".

We will use the Company Contact email address only to contact you about your participation in the Guidescope Affiliate Program. We will not sell your email address to spammers.

Organization or Site Name:
	We will use this name to identify you on the "Customize the Guidescope Menu" form on User Profile pages.
Site URL:
	This is the URL of your site where links to Guidescope.com will appear.
Company Contact Email Address:
	We will use this email address to respond to this application and to contact you about the Affiliate Program.
Guidescope Menu Graphic Location:
	This must be a "gif" or a "png" file on your computer that contains a graphic for the Guidescope Menu that meets the requirements described here.
Guidescope Menu Link URL:
	The URL you want opened when your graphic on the Guidescope Menu is clicked.

By submitting this application you are agreeing to the Affiliate Agreement.
